TERMS OF SERVICE

 

By clicking the Submit button, I acknowledge and agree to the following terms of service:

 

SERVICE GUARANTEE

 

We strive to provide world-class service to our customers and to ensure you are satisfied with those services.  Therefore, if for any reasons you are not completely satisified, please let us know within 24 hours of receiving services and we can schedule a correction within 7 days maximum from the original appointment date. 

 

GENERAL

I understand that I should fully disclose any medical conditions, allergies or prior hair loss issues to my stylist prior to services rendered;  failure to do so may affect the performance of my extensions/braids and result in an adverse reaction.

 

I represent  I have no history or occurrences of adverse skin reaction, whether related to the use of cosmetics or otherwise, and I hereby assume all risks of personal injury which I may sustain as a result from the extension or braids application.

 

I understand fully all extension methods place some stress on the natural hair and that my hair extension/braid service will require proper care and maintenance including regular in-salon and at-home care to eliminate or minimize any potential damage. (*All maintenance guidelines will be fully explained to me prior to services rendered.)   I am aware it is my absolute responsibility to care for and maintain my extensions.

 

I understand that I can return to Kim Nicole HairXtenz for maintenance services (as part of the customer satisfaction guarantee) or go elsewhere for maintenance services but in doing so will void the customer service guarantee.  I understand Kim Nicole HairXtenz does not guarantee the work of others and that the staff, at this salon, is most knowledgeable about my hair extension/braid service for which I am receiving.

 

I understand Kim Nicole HairXtenz uses only 100% Human Hair and as with all natural products variances may occur.  Kim Nicole HairXtenz is not responsible for any naturally occurring variances in the human hair product.

 

Therefore, I do hereby release Kim Nicole HairXtenz and its employees and suppliers/manufacturers from all claims, cause of action or liability that may arise from any and all injuries I may incur as a result of the services performed and/or chemicals used in conjunction with the applications of products furnished to use on or applied to my hair and scalp.

DEPOSITS

I UNDERSTAND THAT ALL CONSULTATION APPOINTMENTS ARE FREE, WITH NO OBLIGATION

** I understand that in order to secure any extension installation appointment, I am required to provide a non-refundable deposit of 50% of the total salon service; including the extension hair purchase, if applicable, at least 72-hours prior to my appointment.  The deposit is required to:  A) cover the expense of the hair to be used during installation, and/or B) hold my appointment. **


I have the right to purchase my own hair on my behalf and in that case, the deposit amount required will be 50% of the installation fee only.


I understand the remaining 50% balance will due at the time of my appointment.

 

I understand I am allowed a 'One-Time Rescheduling Courtesy' and will not forfeit my deposit; provided I have given proper 24-hour notification and I rescheduled and held my appointment within 15 days of the original appointment date.  I understand no other courtesis will be given for cancellations or no-shows.

 

I understand that the deposit terms will be fully disclosed to me during the consultation and that I may request the terms, in written form, at any time.

I understand that if I fail to pay the deposit within 24-hours of booking an installation appointment, Kim Nicole HairXtenz will not hold my appointment in the calendar.

I understand that I have the right to cancel an appointment for any reason and agree to inform Kim Nicole HairXtenz of the change at least 24-hours before the scheduled appointment to avoid forfeiting my deposit.

I understand that by checking this box I agree to all deposit terms listed above.

CREDIT CARD INFORMATION

KIM NICOLE HAIRXTENZ PRACTICES A 24-HOUR CANCELLATION POLICY AND REQUIRES CURRENT BILLING INFO TO BE ON FILE FOR EACH CLIENT.  IN THE EVENT OF A NO SHOW OR LATE CANCEL, I UNDERSTAND THAT I MAY BE CHARGED A CANCELLATION FEE AND/OR FORFEITURE OF DEPOSIT.
